Survey Exercise--Access
Create a new web in FrontPage.  Name it Survey.
Create a form on index.htm.

Include one-line textboxes for:

  • First Name
  • Last Name
  • ID#
  • Favorite Teacher

Include a text area box for:

  • Why Favorite Teacher?  

Name each form field.

Format the background color, font, size and color of your text.  Include ClipArt.

Create your database.  Verify your database.  Update your database.

Post it.  Close FrontPage.  

Test your form.

Your database must have 4 entries.  Yours and three others from class.
Copy and Paste your table.  Rename it: Lastname Results
Format your new table in datasheet view.
Sort it by the last name field.
Print your table.
Create 3 filters.  (filter by form)  Save each with an appropriate name.  Include your last name in the file name.  Print your results.
Create a report that includes all the fields in your database.  Include grouping, sorting, etc.   Name it appropriately and include your last name.  Print the report.
Create a report for one of your three filters.  Apply color to the body of the report.   Name it appropriately and include your last name.  Print the report.
